Why Filling Prices Vary — and What "2 Surfaces" Actually Means

A dental filling is the most common restorative procedure in dentistry — and also one of the most variable in price relative to its perceived simplicity.

"Surfaces" is the key pricing unit. A tooth has five surfaces: the top (occlusal), front (facial), back (lingual), and two sides (mesial and distal). A D2392 filling covers two of these surfaces — a moderate cavity. The number of surfaces affects both the price and how much healthy tooth structure is involved.

Composite vs. amalgam is still a relevant choice. Tooth-colored composite fillings cost more than silver amalgam fillings. Most patients choose composite for aesthetic reasons, and most practices have moved predominantly to composite. If you're quoted for a filling without the material being specified, ask.

Anesthesia is typically included in a filling quote but occasionally billed separately. Clarify whether numbing is included in the price.

What to Watch For

If a routine exam finds multiple cavities requiring fillings — especially if you've had recent dental care elsewhere with no cavities noted — ask your new dentist to show you each cavity on the X-ray or with a camera image. Legitimate cavities are visible and documentable. A dentist who can't or won't show you visual evidence of each cavity before drilling is a red flag.
